MEDICAL & DENTAL HISTORY FORM
As a new patient, we need to get to know you as well as your medical and dental history so that we can gain an understanding of your current and past oral health and denture past so we can provide you with the treatment right for you. For this reason, we will request that you complete a New Patient Form. This can be done in just a few minutes at our practice, prior to your appointment.
However, for your convenience, we have also made this form available as a online form and as a PDF so you can download the form to complete at a time that suits you. Then simply submit the the form online or bring it to your appointment.
We're more than happy to help you complete any sections you find confusing or answer any question you may have.
Of course, any information that you provide will be held in the strictest confidence and is only used by our team to help facilitate the best possible treatment outcomes for you.
